MEDICAL RECORDS REVIEW AND DBQ FOR 1 DIAGNOSIS

This includes a Medical Records review up to 250 pages and 1 DBQ (Disability Benefits Questionnaire) filled out up to 10 pages. There will be an extra charge of $1 per page for Medical Records over 250 pages. There will be an extra charge of $25 per page for DBQs over 10 pages. The Veteran must provide the doctor with his or her Medical Records. We will review VA medical records and Civilian medical records to determine if there is a LINK between the Veteran's disability and their military service. We will provide the Veteran with a typed out DBQ (Disability Benefits Questionnaire) for the Veteran to submit to the VA along with the Nexus Letter for their Disability claim. The Nexus Letter must be purchased separately. We review records and fill out DBQs for Veterans who served in all branches of the US Military, including US Army, Navy, Air Force and Marines. Nexus Letters increase the chances of the Veteran receiving a Disability Rating and Disability Benefits but is NOT guaranteed. Even if the VA has denied the Veteran's Disability claim, the Veteran can APPEAL their decision and submit a NEXUS LETTER to increase the Veteran's chances of approval. If the Veteran has been denied disability benefits for a diagnosis on the "presumed list" for the PACT ACT after serving in DESERT STORM or GULF WAR, the Veteran can APPEAL the VA's decision and submit a NEXUS LETTER to try to get approved.
